Your Role and Responsibilities
As a Registered Nurse in our hospice care team, you will play a vital role in providing compassionate, patient-centered care while supporting patients and their families through their journey.
Patient Care Delivery
Coordinate and provide holistic patient and caregiver care.
Conduct comprehensive nursing assessments to determine patient needs.
Develop, implement, and regularly update individualized care plans.
Foster patient and caregiver independence whenever possible.
Utilize established patient-related protocols and guidelines to ensure quality care.
Education & Communication
Educate patients and families on care procedures and expectations.
Encourage open dialogue by providing time for questions and discussions.
Maintain clear and effective communication with interdisciplinary team members, patients, caregivers, and other healthcare professionals.
Keep leadership informed of complex patient or caregiver concerns.
Interdisciplinary Collaboration
Actively participate in interdisciplinary group (IDG) meetings and special care conferences.
Foster strong professional relationships within the care team.
Identify specific patient or caregiver needs and coordinate with appropriate team members.
Documentation & Compliance
Maintain accurate, thorough, and timely clinical documentation per industry standards.
Follow infection control protocols and universal precautions to ensure safety.
Stay up to date on licensure, certification, and continuing education requirements.
Additional Responsibilities (For Inpatient Unit - IPU Nurses)
Maintain cleanliness and organization of patient rooms and common areas.
Ensure supplies are stocked and appropriate cleaning protocols are followed.
Use protective equipment when handling cleaning chemicals.
What We're Looking For
Education: Diploma or Associate's Degree in Nursing required; Bachelor's Degree preferred.
Licensure: Active, unrestricted compact (multistate) Registered Nurse (RN) license.
Certification: Current CPR certification required; CHPN preferred.
Experience: Minimum of 1 year for inpatient unit nurses and 2 years for other nursing positions.
Experience in hospice, palliative care, geriatrics, oncology, home health, or related fields is required.
Technical Skills: Basic computer proficiency.
Soft Skills: Strong time management, crisis response, and teamwork abilities.
