### Job Overview: Community Health Worker (USC Only)
#### **Location:**
- **Remote Work** to start, but candidates must be located in **Lancaster, York, Dauphin, or Cumberland counties in Pennsylvania.**
#### **Schedule:**
- **Monday to Friday** with occasional weekend work once every quarter.
- **Travel:** Over 50% of the time (travel expenses will be reimbursed).
### **Key Responsibilities:**
1. **Outreach and Education:**
- Conduct face-to-face outreach focused on preventive health, maternity care, care transitions, and chronic conditions.
- Inform members about preventive health services and assess for additional care management needs.
- Make referrals to appropriate services, including behavioral health.
2. **Member Support:**
- Build supportive relationships with members and community centers to empower active participation in health and wellness.
- Assist members with health assessments, identifying barriers to care (like transportation and financial issues), scheduling appointments, and answering questions.
3. **Collaboration:**
- Work with human service providers (e.g., Head Start, WIC, community centers) and state agencies (e.g., Children and Youth Services) to identify and reach out to members requiring services.
- Understand the organization's benefits and services to accurately inform members.
4. **Educational Contributions:**
- Help develop educational materials for members and providers.
- Conduct training for providers and community members on specific health initiatives.
- Report training outcomes to management.
5. **Program Development:**
- Participate in creating effective and innovative programs aimed at at-risk members.
- Represent the organization at community events.
### **Qualifications:**
- **Basic:**
- High School Diploma/GED.
- Must obtain Act 33/34 and 73 clearances prior to starting.
- Proficient in customer service principles.
- **Experience:**
- Medical assistant experience.
- Call center experience.
- **Skills:**
- Strong teamwork and problem-solving skills.
- Ability to multitask and respond promptly to customer inquiries.
- Excellent written and verbal communication skills.
- Cultural competency, particularly with Medicaid populations.
- **Personal Attributes:**
- Patience and empathy in member interactions.
### **Preferred Qualifications:**
- Background in health care customer service or marketing, enhancing member engagement.
- Bilingual (Spanish speaking).
- Familiarity with basic medical terminology.
- Experience with targeted communities or populations.
